This puzzle box was screen-used in Hellraiser: Bloodline, one of the earlier installments in the Hellraiser franchise.  COA from Gary Tunnicliffe, who made the box for the film. The box was purposely aged and distressed for filming.  It's made from solid wood with brass etchings, and has a nice weight to it.
